HEALTH EQUITY PROGRAM MANAGER

Position Description

This position is responsible to support, coordinate, develop and lead UCare's efforts to advance health equity. Support the progress toward UCare being the leader in providing all members a path to the best health for their lives with the goal of becoming the health plan leader in addressing the integration of social and medical needs. Important in this strategy is uncovering lack of health equity or the presence of health disparities in the members and communities UCare serves. Collaborate with existing UCare areas and have a dotted line reporting relationship to the Senior Vice President, Public Affairs and Chief Marketing Officer.

• Coordinate the planning, development and implementation of UCare programs to address health disparities, and advance health equity and community health. Collaborate with areas across UCare to incorporate existing resources and exploration of new opportunities.

• Support and collaborate internally with population health, community relations, and other areas whose work involves addressing health disparities, and advancing health equity and community health.

• Equip UCare leaders with an understanding of health disparities, health equity and community health.

• Support UCare leaders to develop strategies, tailored to their areas, that address health disparities, and advance health equity and community health.

• Develop and maintain an inventory of community-based and governmental organizations and resources for UCare collaboration on health disparities, health equity and community health. Develop processes for the use of this inventory internally.

• Connect with external groups that are addressing health disparities, health equity and community health.

• Coordinate UCare's work with members, communities and outside organizations to understand and co-develop solutions to address identified gaps in health equity.

• Identify existing and create forums or opportunities for UCare and community members who have been impacted by inequities to come together, describe their experiences and challenges and participate in co-creation of solutions aimed at reducing disparities.

• Organize a network of leaders that are addressing health disparities and advancing health equity and community health to inform the work of UCare and other health plans.

• Facilitate the data collection, tracking and reporting of outcomes for UCare strategies to address health disparities, and advance health equity and community health

• Maintain knowledge of industry trends and practices.

• Other projects and duties as assigned.

Education

Bachelor's degree in public health, social sciences, or other health care related field required. Master's degree preferred.

Required Experience

Five years of experience working in the field of public health, health disparities, and health equity. Experience working cooperatively and collaboratively with diverse internal and external stakeholders. Program management experience.

Preferred Experience

Experience working in the field of community health improvement.
