CONNECT ASSESSOR (TEMPORARY - 3 MONTH ASSIGNMENT)

Position Description

This position is responsible for completing face-to-face assessments for our health plan members on UCare Connect. Evaluate UCare members to determine current health status needs, assign appropriate case management level of care, and document findings in UCare's internal tracking system and the State of Minnesota's system. After training, these positions will Work-at-Home and will be required to come into the office on occasion. This position requires reliable transportation for travel throughout the metro and surrounding counties.

• Complete face-to-face assessments for UCare Connect members throughout the metro area and surrounding counties.

• Evaluate member current health status needs, assign appropriate case management level of care and document findings in UCare's internal tracking system and the State of Minnesota's system.

• Other projects and duties as assigned.

Education

B.S. in nursing or BA in social work. Current and unrestricted MN license in field required.

Required Experience

Three years' experience in care coordination/Case management across the continuum of health care (hospital, clinic, nursing home, home care, etc.) with primary emphasis in working with clients with complex medical issues and/or disabilities. Strong assessment skills. Proficient computer skills are necessary, along with the ability to work independently with minimal supervision.

Preferred Experience

Knowledge regarding case management, managed care organizations, and home and community-based services. Experience working with multi-cultural and variable socioeconomic populations. Experience working for a health maintenance organization serving Medicaid members.
