This full-time position will support adults with serious and persistent mental illness primarily Monday-Friday during business hours (8:00-4:30) through Targeted Mental Health Case Management. The focus of these services is to help individuals thrive in their communities; maintaining stability and maximizing self-sufficiency. This includes gaining access to needed medical, social, educational, vocational, and other necessary services. The Case Manager completes a functional assessment and helps individuals develop a person-centered community support plan, ensures coordination of supports, and monitors the delivery of services. This position is community-based with a Shakopee-based office.

MQs: Requires equivalency of a bachelor's degree in a human service-related field and two years of professional social work experience working directly with clients. Strong preference for experience providing/coordinating direct service to adults with serious and persistent mental illness. Master's degree and bilingual abilities are desirable. Must possess a valid driver's license and provide a reliable means of transportation for the performance of work responsibilities. 

2020 Hiring Range: $61,484 to $76,855 - DOQ
