CARE MANAGEMENT TEAM MANAGER - I-SNP

Position Description

This position is responsible for oversight of the Institutional Special Needs Plan (I-SNP). Manage or influence matrixed UCare services to assure clinical I-SNP operations meet regulations and support the approved I-SNP model of care (MOC). They will also be responsible for ongoing CMS approvals of future I-SNP MOC renewals. Oversee and influence I-SNP Care Delegates to assert a grounded, sufficient program (including care coordination, primary care, institutional facilities and vendors). Contribute to evaluations of new I-SNP Care Delegates and champion clinical onboarding. Evaluate I-SNP clinical activities for members to ensure quality service, and compliance with contractual and regulatory requirements. Responsible for all documented content (policies, procedures), quality improvement initiatives, and measured results. Manage I-SNP clinical care system liaison(s) operation and performance. Collaborate with internal stakeholders to grow, evolve, stabilize and successfully position the clinical aspects of the I-SNP program.

• Exercise expertise in the I-SNP product and care model, internally at UCare and nationally, to inform best clinical practice and maintain the I-SNP's good standing in the Model of Care with CMS. Oversee all Model of Care renewal activities with CMS.

• Establish strong relationships with I-SNP Care Delegates. Monitor and evaluate clinical performance of the I-SNP Care Delegate(s), regularly addressing variances to expectations. Contribute to the evaluation for potential I-SNP Care Delegates, institutions and vendors. Orient and clinically collaborate with new I-SNP Care Delegates.

• Ensure regular and timely maintenance of all I-SNP documentation for compliance with all regulatory and contractual obligations.

• Monitor, analyze and engage to address product performance and improvement opportunities. Identify and propose ways to simplify, streamline and reduce unnecessary costs of the product, both internally and externally.

• Oversee assigned ISNP clinical care system liaison team in the performance of care coordination oversight including: New and ongoing delegate training, delegate enrollment processes, materials development and compliance with regulatory requirements. Develop and maintain processes to provide support and communication processes for ISNP care coordination delegates and providers, as appropriate. Coordination and facilitation of care coordination-related projects as directed, such as monthly enrollment files, implementing a new product, etc.

• Provide training, supervision, and staff development to enhance the knowledge and skills of I-SNP clinical liaison staff. Train staff on requirements for assigned product(s) and corresponding processes, establish regularly scheduled one to one staff supervision at a minimum frequency of monthly to review and evaluate staff performance, establish regularly scheduled team meetings, assist with problem solving. The I-SNP manager will collaborate with the Clinical Operations Manager/Director to coordinate new employee orientation and evaluate additional staff development needs as needed.

• Partner with internal stakeholders, contributing clinical thought leadership to I-SNP's growth, evolution, stabilization and successful positioning within the marketplace.

• Collaborate with UCare on projects, operations and initiatives, as appropriate.

• Maintain a working knowledge of regulations, contractual requirements and team functions.

• Manage identified community partnerships, key provider relations activities, and associated public relations interactions.

• Other projects and duties as assigned.

Education

Bachelor's degree in nursing or equivalent experience or BA in social work. Current and unrestricted license required.

Required Experience

Five years' experience in the health care industry. Two years' experience in case management across the continuum of health care (hospital, clinic, nursing home, home care). Two years' experience supervising professional staff. Strong health plan, managed care, health services, or other related experience.

Preferred Experience

Experience working within a nursing facility or institution. Experience working with Medicaid or Medicare risk managed programs.
