DUTIES
Identify and assess the nutritional needs of assigned residents in long-term care and transitional care settings along with completing nutritional assessments and documentation according to state and federal regulations.
Initiate interventions to achieve and/or maintain residents' nutritional status and enhance quality of life consistent with resident's goals.
Provide nutritional expertise to the interdisciplinary team and educate residents and staff as needed.
Interact with other departments in setting up procedures, policies and problem solving as they relate to the residents nutritional care.
Develop individualized diet plans for residents.
Ensure accuracy and consistency of diet orders, nourishments and weights on a regular basis.
Manage tube feedings.
Initiate appropriate nutrition interventions utilizing laboratory data, weight changes, disease state and appetite.
Conduct meal rounds as scheduled to evaluate resident satisfaction and quality and appropriateness of meals and service.
Perform other tasks and projects that support the mission of MMH and the Dietary Department, as needed.
Full Time, 70 hours per pay period, Monday-Friday
QUALIFICATIONS
Must possess a Bachelor's degree from an accredited institution with a major in nutrition or dietetics and have completed an approved internship under the auspices American Dietetic Association.
Registered Dietitian with the Commission on Dietetic Registration (CDR)
Licensed Dietitian in the State of Minnesota
Minimum of one year experience in a skilled nursing care facility, hospital, or other related medical facility
Knowledgeable of Nutrition Therapy for all levels of care and disease states
Proficient with electronic charting, Microsoft office, as well as web based programs
