HEALTH EDUCATOR

Work Location: in the office or hybrid (in the office 3 or more days per week)

Position Description

This position is responsible to develop, implement, lead and report outcomes for health promotion programming for assigned UCare member programs. Participate in other member health initiatives. Provide expertise to other UCare departments and initiatives as assigned.

• Develop, implement and oversea UCare's Health Promotion programs for primarily the medicaid and dually eligible medicare and medicad programs.

• Support development of Health Promotion department strategies and reports. Maintain current, accessible and useful methods of documenting and evaluating programs. Compile and prepare reports representing activities, productivity, cost savings and recommendations.

• Identify potential new prevention initiatives and health promotion programs designed to maintain a leadership position in driving member responsibility for their health care.

• Manage UCare efforts in preventative health initiatives; including behavior change and preventative quality health improvement goals. Provide ongoing monitoring, evaluation and issue resolution of assigned projects. Develop key objectives, activities, outcome and process measurement plans. Manage within budget and deliverable dates.

• Work cross departmentally and in collaboration to ensure contracts, payment, communication, etc are in place for successful program management.

• Participate in projects or workgroups to assist UCare quality ratings.

• Provide expertise to key internal and external customers regarding Health Promotion programs and services as assigned.

• Collaborate with public health and community organizations on public health and community initiatives.

• Other projects and duties as assigned.

Education

Bachelor's Degree in Health Education, Public Health or equivalent degree required.

Required Experience

Experience with medicaid populations, child and teen services and car seat education. Experience in a health delivery setting, such as health department, physician group, nonprofit agency, health plan or community clinic including experience in administrative or project management.

Preferred Experience

One year experience working with diverse populations such as Somali, Hmong, Latino, or African American. Health prevention program development experience preferred. Master's degree in Health Education, Public Health or equivalent.
