CommonBond Communities is currently recruiting for Social Service Coordinators for several locations in the Twin Cities. The Social Service Coordinator provides culturally appropriate individual services, programs, assessment, referral, and advocacy to promote resident stability for the seniors living in this community. The Social Service Coordinator also plans, coordinates, and evaluates programs to promote healthy and positive relationships within the community.

ESSENTIAL JOB FUNCTIONS AND ASSOCIATED TASKS

 Plan, coordinate, promote, manage and document the delivery of Advantage Center programs and services provided for the benefit of residents, with a focus on housing stability.

  1. Identify and assess individual and community needs and assets.
  2. Solicit regular resident input and feedback.
  3. Develop and provide programming and services that promote education, employment, financial advancement, and health & well-being for residents.
  4. Develop and maintain partnerships with community providers, link residents to community resources and assist with accessing income supports.
  5. Collaborate with other CommonBond Communities staff to meet program needs.
  6. Keep current with best practices, utilize relevant tools and resources, and attend any necessary trainings relevant to program areas.
  7. Maintain records, complete data entry, and provide information as requested.
  8. Coordinate volunteer participation in the delivery of programs.
  9. Solicit regular resident input for program planning.

 Provide individual services coordination to support residents in maintaining housing at assigned housing communities.

  1. Respond to referrals and requests for services, and conduct outreach to residents.
  2. Conduct New Resident Orientation with new residents.
  3. Provide residents with information, advocacy and support.
  4. Assist residents with housing stability issues such as rent payments, subsidy certifications, inspections, housing conditions and lease violations.
  5. Partner with Property Management staff to ensure resident compliance with housing requirements
    1. Assist residents with inspections and housing conditions
    1. Support residents with lease education
    2. Assist residents with accessing resources to ensure on-time rent payments
  1. Conduct professional interviews with residents and complete necessary documentation for timely resident certifications
  1. Work closely with other Housing and Services staff to develop and maintain safe, stable housing and to promote resident self-sufficiency.
  2. Work with residents to set goals, develop plans, and make referrals and connections with community service providers.
  3. Maintain ongoing communication with referral sources, residents, other CommonBond staff, and community service providers.
  4. Act as a liaison between community-based service providers and housing staff for the benefit of residents.
  5. Maintain accurate, timely and detailed documentation on casework in accordance with professional ethics and organizational procedures.
  6. Create and annually update a directory of relevant community services and resources for use with residents 

Promote Community Building

  1. Develop and facilitate activities, strategies and programming that promote positive community engagement.
  2. Support resident associations, resident-led activities and leadership development by providing technical and resource support.
  3. Develop collaborative relationships to support Advantage Center Operations.
  4. Effectively represent CommonBond Communities in neighboring communities and relevant service networks.

Team Participation

  1. Work collaboratively with the site Housing and Services team.
  2. Participate collaboratively in all meetings and trainings.
  3. Participate in Casework group meetings.
  4. Maintain productive relationships with owner/affiliate boards and community partners.
  5. Participate in corporate projects and complete other assignments as requested.
  6. Participate in ongoing interactions with Resource Development team on volunteer recruitment and placement, donations, and fundraising efforts

 Program Administration

  1. Prepare, monitor or assist with service agreements.
  2. Prepare or assist with reports for owner/affiliate boards or other entities.
  3. Complete all data entry and documentation within designated timelines.
  4. Set annual goals and review goal progress on a regular basis.
  5. Review budgets and relevant data reports on a regular basis.
  6. Review Housing and Services reports (i.e., delinquency, lease violations) on a regular basis.
  7. Create and distribute a monthly newsletter for assigned housing sites.

 Professional Development

  1. Develop professional growth plans.
  2. Contribute to professional growth efforts of the Housing and Services team.
