CARE MANAGER - CHILD HEALTH - PERINATAL

Work Location: Work from home, in the office or hybrid (in the office 3 or more days per week)

Position Description

This position is responsible to coordinate services across the continuum of health care to meet the health and social service needs of identified members focused in the area of Maternal and Child Health. Collaborate with treatment providers, county and community agencies, pilot provider programs and other key stakeholders to identify and coordinate provision of health care services for UCare members.

• Provide telephonic and community-based case management support to members. As appropriate, member support may include clinic and/or patient home visits.

• Understand and accurately interpret and apply relevant contractual requirements, policies, procedures and regulations for those members for which care management is a provided service.

• Partner with members and/or family member, primary care providers, clinic staff and other relevant agencies to assure appropriateness of service that meets member needs and supports desired outcomes.

• Complete comprehensive assessment for assigned members. Collaborate with member/caregiver in the development of a care plan appropriate to the member's needs. SMART (Specific, Measurable, Achievable, Relevant, Time) goals are utilized to create Care Plan(s).

• Identify and monitor member needs, including needed preventive medical care, and significant change in condition which may warrant early intervention for medical problems, or transition of care. Incorporate ethnic and culturally appropriate approaches to case management and care planning as needed.

• Provide health education and coaching on pregnancy and postpartum topics. Offer and complete health and mental health screenings at suggested intervals and refer to appropriate resources.

• Document accurately, thoroughly, and timely in UCare's clinical documentation software member information.

• Meet and maintain all established caseload and performance metrics.

• Attend internal and external meetings; may act as team representative on cross departmental meetings as assigned.

• Assist as necessary with UCare's NCQA Complex Case Management program or other clinical initiatives. May also assist with CMS Star Rating/HEDIS initiatives and project improvement planning as appropriate.

• Support racial and health advancement goals as part of Clinical initiatives and UCare's anti-racism effort.

• Collaborate through team efforts to advance the Maternal & Child Health Case Management program.

• Other projects and duties as assigned.

Education

Registered Nurse (BSN preferred). Current and unrestricted RN license in Minnesota required.

Required Experience

Two years experience in Pediatrics, NICU, and/or OB/GYN care coordination/case management across the continuum of health care (hospital, clinic, home care etc.). Emphasis on working with at risk, diverse populations.

Preferred Experience

Combination of Public Health work with families and children and working knowledge of managed care or commercial insurance environments.
