DISEASE MANAGEMENT PROGRAM COORDINATOR

Work Location: Work from home

Position Description

As the Disease Management Program Coordinator, you will be responsible to provide ongoing support and coordination of assigned Disease Management (DM) programs to ensure the programs are achieving their intended purposes. In addition, serve as a liaison between the Disease Management delegates, vendors, UCare staff, UCare providers, community and members. This role is responsible for the day-to-day operational aspects of Disease Management programs. Activities include, not limited to, tracking, documenting, and reporting work efforts and tasks related to DM program implementations.

• Assist in the management of all aspects of DM programs including development, implementation, reporting and evaluation of new and existing DM programs.

• Coordinate transfer of data, member referrals and member follow up between UCare and DM delegates and vendors.

• Coordinate member outreach activities including scheduling member mailings and outbound IVR calls promoting DM programs.

• Manage calls, faxes and emails coming into DM and provide or assign to staff for follow up. Enter member information into the clinical documentation software. Complete accurate, thorough, and timely outcomes.

• Manage DM program materials and lead process for updating materials. Provide maintenance of all DM program documents in SharePoint or designated company site. Monitor DM program materials inventory.

• Represent DM as the clinical documentation system Subject Matter Expert (SME). Manage all aspects of DM materials in the clinical documentation system and attend work group meetings as applicable.

• Assist with annual DM program activities and audits such as the annual DM program member satisfaction survey and NCQA audits.

• Develop and implement strategies to reach and maintain DM program enrollment goals. Prepare and/or monitor routine participation reports. Collect and compile data to be used towards addressing DM goals and compliance standards. Maintain a tracking system for department outreach activity including standard and ad-hoc reporting requests.

• Serve as the DM SME to UCare staff, providers and members enrolled in DM programs. Provide ongoing education and information about DM programs. Communicate program updates as requested.

• Collaborate with UCare teams including IT, Health Care Economics, Quality Management and Clinical Services to document, schedule and identify project timelines, milestones, resource allocation and problem-solving strategies. Serve as a DM representative at internal and external meetings and committees as appropriate.

• Other projects and duties as assigned.

Education

Bachelor's degree; demonstrated experience may be considered in lieu of degree.

Required Experience

At least two years of project management experience in a health plan environment. Experience in disease management or chronic disease care. Proficiency in the use of Microsoft Excel, Project and other Office applications.

Preferred Experience

Two years of working in the managed care industry with an emphasis in project management.
