SNBC PROGRAM MANAGER

Work Location: Work from home

Position Description

As the SNBC Program Manager, you will be responsible for the program management of UCare's Special Needs Basic Care (SNBC) products. This position will assist with setting priorities for the SNBC care coordination programs, including process improvement strategies, organizational goals, regulatory guidance and data. Interpret and apply state and federal regulatory mandates to support regulatory readiness and accreditation. Oversee the implementation of the priorities to ensure contractual, and regulatory requirements are communicated and understood by stakeholders. In addition, you will also be responsible to maintain relationships with delegated entities to assist in positive collaboration and open communication.

• Oversee team in the performance of care coordination and care navigation activities including compliance with all contractual obligations

• Monitor delivery of care coordination services. Review and evaluate care coordination and care navigation staff documentation of assessments, care plans, and care coordination activities, audit member records, monitor staff productivity, identify system barriers and possible solutions.

• Provide training, supervision, and staff development to enhance the knowledge and skills of care coordination and care navigation staff. Establish regularly scheduled one to one staff supervision at a minimum frequency of monthly to review and evaluate appropriateness of care coordination services and care navigation services and activities, establish regularly scheduled team meetings, assist with problem solving complex member care issues, conduct supervisory oversight of staff/member interactions.

• Work collaboratively with Clinical Care System Liaisons, as appropriate to ensure communication member needs are met. Provides clinical consultation to clinical liaisons working with delegate staff as needed.

• Assist with the oversight of regulatory reporting for SNBC products: Complete the MOC Evaluation annually, work with Compliance on annual DHS reporting, work with the Director on writing and submitting the MOC to CMS. Prepare and submit regulatory findings to stakeholders.

• Oversee the Care Coordination Grids. Work with the clinical liaisons to update the language while providing sound reasoning for updates that can be explained and implemented in a timely way, including allowing time for training to occur, and for supportive documentation to be publicly posted for delegates to access.

• Develops tools and resources to assist the care coordinators in performing their duties, including assessments, care plans, training opportunities and algorithms for commonly occurring medical and social complexities in the population.

• Collaborate with the Quality Management team on HEDIS ratings, Stars Ratings measures, clinical outcome measures, strategies, and interventions, as appropriate.

• Collaborate with Health Care Economics (HCE) on tracking care coordination measures and impacts on overall total cost of care, including interventions in specific areas, and with specific demographics,

• Participate in developing and implementing UCare strategy for care coordination in new markets and new products.

• Build and leverage effective cross-functional relationships, influence and support a culture of compliance.

• Successfully manage relationships and coordinated efforts with delegated vendors as assigned.

• Participates in the development of department policies and procedures for the SNBC program.

• Participate in meetings as assigned by the Director.

• Other projects and duties as assigned.

Education

Bachelor's degree in nursing or social work or licensed mental health professional. RN diploma or Associate RN degree plus case management certification may be considered. Current, unrestricted, Minnesota license required.

Required Experience

Five years supervisory or management experience in a health plan, managed care organization, or clinical setting. Experience with program performance measurement, data, reporting, and analysis. Knowledge of State and Federal regulations, experience with national accreditation standards.

Preferred Experience

Experience working with disabled, multi-cultural and underserved populations desired.
