Work Location: Work from home

Position Description

This position is responsible to support initiatives related to pharmacy clinical quality measures including Stars, HEDIS, population health, and other programs as necessary. Work to help improve transitions of care and promote safe and effective medication use. Responsible for medication reconciliation services and/or comprehensive medication reviews to members who have recently discharged from the hospital. Collaborate with other UCare departments, pharmacists, providers, care coordinators, social workers, vendors, and other external parties to ensure patient needs are met.

• Complete telephonic or in-home medication reconciliation services to members who have recently transitioned from the hospital.

• Comprehensively review all medications. Identify, document, and facilitate resolution of any identified drug therapy problems (DTPs).

• Counsel patients about any medication changes and provide educational materials as needed.

• Conduct outreach for members not achieving clinical goals.

• Maintain required documentation for all care management, medication reconciliation, and comprehensive medication review activities.

• Collaborate with internal departments and external stakeholders for improvement in pharmacy-related quality initiatives.

• Provide oversight and mentor pharmacy students and/or pharmacy interns and guide them in completing assigned projects.

• Other projects and duties as assigned.


Bachelor of Science in Pharmacy or PharmD required. Current and unrestricted Minnesota pharmacy licensure required.

Required Experience

Two years of clinical pharmacy practice experience or residency training. Experience providing clinical care or MTM services.

Preferred Experience

Residency training and pharmacy board certification in an applicable specialty (e.g., ambulatory care, pharmacotherapy, or geriatric). Bilingual proficiency.
