Social Work Case Manager – Adult Mental Health

This full-time position will support adults with serious and persistent mental illness primarily Monday-Friday during business hours (8:00-4:30) through Targeted Mental Health Case Management. The focus of these services is to help individuals thrive in their communities, maintaining stability and maximizing self-sufficiency. This includes gaining access to needed medical, social, educational, vocational, and other necessary services. The Case Manager completes a functional assessment and helps individuals develop a person-centered community support plan, ensures coordination of supports, and monitors the delivery of services to adults with serious and persistent mental illness and adults experiencing housing instability. This position is community-based with a Shakopee-based office.

MQs: Requires equivalency of a bachelor's degree in a human service-related field and two years of professional social work experience working directly with clients. Strong preference for experience providing/coordinating direct service to adults with serious and persistent mental illness. Master's degree and bilingual abilities are desirable. Must possess a valid driver's license and provide a reliable means of transportation for the performance of work responsibilities.

Hiring Range: $64,754 to $80,943 - DOQ
