MEMBER ENGAGEMENT SPECIALIST - MSHO/MSC+ REFUSAL-UNABLE TO REACH

Work Location: Work from home

Position Description

This position is responsible to provide outbound telephonic engagement calls to engage unable to reach and refusal MSHO and MSC+ members to improve care coordination program participation. You will focus on engaging, recruiting and enrolling members into care coordination programs.

• Provide outbound telephonic engagement calls to members who have been unable to reach or previously refused to engage in care coordination.

• Perform daily administrative duties to identify members eligible for care coordination programs.

• Collaborate with teams to document, schedule and identify project timelines, milestones, resource allocation and problem-solving strategies.

• Process daily and monthly outreach lists, referrals and inbound care coordination phone calls.

• Utilize motivational interviewing skills to engage members in conversations about the benefits of care coordination and their health-related needs.

• Collaborate with the CLS Oversight and Compliance Manager and Supervisor to update member contact lists.

• Provide outbound telephonic engagement calls to members with identified gaps in care to assist in obtaining preventive care and services.

• Identify and implement strategies to reach and maintain Care Management (CM) program goals.

• Complete productivity tracking as directed

• Collaborate with members and/or family members, providers, clinic staff, and other relevant agencies to determine the best source of communication services that meet member needs and ensures desired outcomes.

• Enter member information in the clinical documentation software. Complete accurate, thorough and timely documentation.

• Maintain good working relationships and open communication with internal and external customers.

• Other projects and duties as assigned.

Education

High school diploma or equivalent, graduate or education from an accredited college, university or vocational school.

Required Experience

Two years of customer service or sales and marketing experience in a call center, health insurance, physician group practice, or community agency. Proficient PC skills and in Microsoft Office (with emphasis on Excel spreadsheets).

Preferred Experience

Knowledge of and experience in telephonic outreach coordination. Excellent computer word processing and data base experience. Experience with MMIS and/or Electronic Health Record (HER) preferred.
