I-SNP PROGRAM COORDINATOR

Work Location: Work from home, but must be within a commutable distance to attend in-person meetings as necessary.

Position Description

This position is responsible for fostering and developing strong partnerships and serving as a key contact for delegate inquiries about UCare's Institutional Special Needs Plan (I-SNP) products and care coordination processes including changes in process and regulatory expectations. Participate as a key contributor in clinical training of UCare's care system as well as in the planning, implementation, and evaluation of I-SNP product enhancement.

• Serve as main contact and provide support for delegates with clinical issues as they relate to I-SNP.

• Represent I-SNP in internal and external care system work groups and initiatives as directed by the manager.

• Identify knowledge gaps with key stakeholders and identify the need for training. Utilize audit findings to identify knowledge gaps with external customers and assist with closing identified knowledge gaps. Conduct training as needed for care coordination partners, including model of care training. Provide in-person, live training whenever possible.

• Develop and maintain care coordination training materials for I-SNP delegates, including product information and updates, care coordination manuals, care coordination requirements grid, and care management specific webpages, etc.

• Maintain a strong working knowledge of care coordination functions, requirements, and regulations. Subject matter expert in care coordination responsibilities, which includes responding to questions related to care coordination functions independently.

• Maintain a working knowledge of CMS regulations, and programmatic functions.

• Coordinate and facilitate care coordination-related projects and meetings as directed, such as participating in the Quarterly Operational Meetings, creating, or maintaining reports, responding to information requests by regulators, etc. Be able to prepare and implement a project plan, statements of work, and/or process flow related to different projects, and be able to prepare and report results analysis as needed.

• Collaborate with team and provide back-up as well as maintain a positive working relationship with delegate staff; maintain those relationships with finesse and graciousness.

• Prepare monthly reports on activities and identify opportunities to increase efficiencies, productivity, and quality of work. Be knowledgeable about the demographics that delegate agencies work with and be prepared to support them with demographic specific resources, including social determinants of health. Analyze monthly reports on activities and identify opportunities to increase efficiencies, productivity, and quality of work.

• Other projects and duties as assigned.

Education

Bachelor's degree in social work or nursing. Current unrestricted Minnesota license.

Required Experience

Two years' experience in training/leading or facilitating discussions. Strong health plan, managed care, health services, or other related experience. Ability to present to groups, knowledge of project coordination.

Preferred Experience

One year experience in Medicaid or Medicare risk managed care programs, as well as with aging populations.
