Job Details

This Incumbent performs a variety of activities and tasks in implementing the Statewide Health Improvement Partnership (SHIP) Grant aimed at improving preventative care, policy, and systems strategies to reduce long term care costs related to obesity and tobacco use.

Implements and evaluates a comprehensive, unified chronic disease prevention plan, public health policies and programs for the SHIP and SHIP-related areas for Scott County.

Develops and conducts needs assessments and evaluation plans incorporating evidence-based strategies using qualitative and quantitative assessment methods, analyzes results and writes needs assessment reports.

Develops and strengthens partnerships in the community. Advocates for policy, systems and environmental change related to grant project.

Staffs Healthy Eating Active Living Action Team and acts as a liaison between the community and the Community Health Improvement Committee.

Provides technical assistance to community partners on chronic-disease and SHIP-related matters.

Works with SHIP Coordinator and other assigned staff and contractors as part of the SHIP project team.

Provides emergency / disaster public health services, including emergency preparedness activities as required.

MQs: Requires equivalency of a bachelor's degree in Public Health, Community Planning, Public Policy or related field and two years experience with public health, health promotion or coaching, community based planning and organizing. Experience with community and systems interventions or health promotion strategies and evaluation preferred. One must possess a valid driver's license and a reliable means of transportation for the performance of work responsibilities.

Hiring Range: $53,369 to $66,711 - DOQ
