COMPLEX CARE MANAGEMENT (CCM) CARE MANAGER & TRAINER

Work Location: Office for Your Day

Position Description

As the Complex Care Management (CCM) Care Manager & Trainer, you will split your time between Care Management (CM) training and development and CCM Case Management to UCare members. You will be responsible for maintaining a reduced caseload while also maintaining training and development material, facilitating new hire CCM training, supporting organizational training standards templates and supporting streamlined work processes. Collaborate with internal departments such as Learning & Development and Quality to improve training, gather materials, research, and develop CM learning content to support NCQA accreditation standards, onboarding, and ongoing nursing knowledge as needed.

• Provide telephonic case management; including: engagement, assessment and development, implementation and evaluation of their plan of care. As appropriate, member support may include clinic and/or patient home visits.

• Understand and accurately interpret and apply relevant contractual requirements, policies, procedures and regulations for those members for which care management is a provided service.

• Partner with members and/or family member, primary care providers, clinic staff and other relevant agencies to assure appropriateness of service that meets member needs and supports desired outcomes.

• Complete assessment of assigned members. Collaborate with member/caregiver in the development of a care plan appropriate to the member's needs. Care plan goals are measurable, specific and timebound.

• Identify and monitor member needs, including needed preventive medical care, and significant change in condition which may warrant early intervention for medical problems, or transitions of care. Incorporate ethnic and culturally appropriate approaches to care planning as needed.

• Enter member information in UCare's clinical documentation software. Documentation is accurate, thorough and timely.

• Meet and maintain established caseload and performance metrics.

• Attend internal and external meetings; may act as team representative on cross departmental meetings as assigned.

• Assess and develop training solutions that provide relevant, engaging, and productive learning experiences to new hires.

• Assist with development of job aids and material to support team with clinical documentation system.

• Collaborate with department leadership in on-boarding and integration process of new staff; ensure timely and accurate delivery or training programs for new and existing staff to support NCQA accreditation standards and CM effectiveness.

• Provide group or one-on-one training to CM team as needed to support departmental workflow changes, strategic efforts, NCQA or other identified learning needs.

• Maintain tracking system to maintain timely completion of CCM onboarding and future trainings for employees.

• Assess training outcomes, identify and implement improvements; develop reporting measures of training needs and results through surveys, evaluations forms, or consultations with leadership staff.

• Collaborate as needed with the Corporate Learning and Development Team to ensure best practice and organizational standards are implemented.

• Assist with UCare's NCQA Complex Case Management program and other clinical initiatives, as needed. May also assist with CMS Star Rating/HEDIS initiatives and project improvement planning as appropriate.

• Other projects and duties as assigned.

Education

BSN Registered Nurse Required/ Master's Degree preferred. Current and unrestricted RN license in Minnesota required.

Required Experience

Two years experience in care coordination/case management across the continuum of health care (hospital, clinic, home care etc.) with primary emphasis working with chronic medical conditions. Experience with training and training program development and adult learning principles.

Preferred Experience

Managed care or commercial experience. Supervisory or team lead experience preferred, with specific background in coaching/mentoring.

Experience with process improvement.
