CARE COORDINATOR TEAM MANAGER - SNBC

Work Location: Office for Your Day

Position Description

As the Care Coordinator Team Manager - SNBC, you will be responsible for managing the internal care coordination program of UCare's Special Needs Basic Care (SNBC) products. This position will assist with setting priorities and facilitating implementation of those priorities for the SNBC internal care coordination programs, including process improvement strategies, organizational goals, meeting contractual and regulatory requirements, and data collection. Interpret and apply state and federal regulatory mandates to support regulatory readiness and accreditation. Ensure regulatory requirements are communicated and understood by internal and external stakeholders.

• Oversee teams' performance of care coordination and care navigation activities including compliance with all contractual state and federal obligations.

• Monitor delivery of care coordination services. Provide direction and oversite to care coordination and care navigation staff's documentation of assessments, care plans, and care coordination activities, audit member records, monitor staff productivity, identify system barriers and recommend solutions.

• Oversee SNBC Care Coordinator supervisors to ensure proper staff supervision is in place. In the absence of care coordinator supervisor, assigns and allocates workload to care coordinators. Establish regularly scheduled one-on-one meetings with direct reports at a minimum of once a month and facilitate team leadership in conjunction with other SNBC leaders to ensure that there is uniformity and consistency in delivery of the care model.

• Support SNBC supervisors in hiring additional staff to meet established hiring goals.

• Assist with coordinating the SNBC redesign efforts and identify potential impacts to the internal team(s) within UCare.

• Assist with the oversight of regulatory reporting for SNBC products: support other SNBC leaders in the completion of the annual Model of Care Evaluation, work with the Compliance Department and Government Relations on annual DHS reporting, assist the SNBC Director with writing and submitting the Model of Care to CMS. Prepare and submit regulatory findings to stakeholders as needed.

• Develops tools and resources in collaboration with the SNBC supervisors to assist the care coordinators and care navigators in performing their duties, including assessments, care plans, training opportunities and algorithms for commonly occurring medical and social complexities in the population.

• Provide leadership and guidance related to overall enhancements and changes to the program as directed by CMS, DHS, and/or internal UCare changes. This may include such things as working together to provide complex care coordination for members who may have unique needs for support in their utilization of the health care system, and strategizing about member specific interventions, and facilitation of same.

• Assist with internal alignment of care coordination services and expectations between departments.

• Collaborate with the Quality Management team regarding HEDIS measures, CMS Stars Ratings, clinical outcome measures, strategies, and interventions, as appropriate.

• Collaborate with Health Care Economics (HCE) on tracking care coordination measurements and impacts on overall total cost of care, including interventions in specific areas and with specific demographics.

• Participate in the development and implementation of UCare strategies related to care coordination in new markets and new products.

• Build and leverage effective cross-functional relationships, influence and support a culture of compliance.

• Successfully manage relationships and coordinate efforts with delegated vendors as requested and in accordance with the other leaders on the SNBC team.

• Participates in the development of department policies and procedures for the SNBC program.

• Recruit, interview and select new staff.

• Participate in meetings as assigned by the SNBC and Connect Plus Director.

• Other projects and duties as assigned.

Education

Bachelor's degree in nursing or social work or licensed mental health professional. RN diploma or Associate RN degree plus case management certification may be considered. Current, unrestricted, Minnesota license required.

Required Experience

Five years supervisory or management experience in a health plan, managed care organization, or clinical setting. 2-3 years of care coordination and/or case management experience. Experience with program performance measurement, data, reporting, and analysis. Knowledge of State and Federal regulations, experience with national accreditation standards.

Preferred Experience

Experience working with disabled, multi-cultural and underserved populations desired.
