Ecumen Pathstone is hiring a Registered Nurse RN and/or Licensed Practical Nurse LPN. Ecumen Pathstone is a campus community offering comprehensive care services. Ecumen Pathstone consists of assisted living, long-term care, rehabilitation care, memory care, adult day services, and independent living. Ecumen also offers homecare, hospice, as well as a supply store- all within the Mankato, MN area.

Schedule: Full Time Overnight NOC Shift + every 3rd weekend rotation

Responsibilities

The role of the Registered Nurse (RN) is to ensure that the health and safety needs of all residents are met. The RN will also help ensure that the facility is in compliance with current regulations and standards of practice. Must be able to relate professionally with residents, staff and family members.

The Licensed Practical Nurse (LPN) is responsible for providing nursing care to residents, including medications and treatment administration, documentation, and other therapeutic interventions under the direction of the Director of Nursing/Clinical Director. Provides direction to nursing assistants, resident assistants, trained medication assistants, and other clinical staff on unit/shift.

Wage: $27.00 - $36.00 depending on work experience



Essential job responsibilities include RN:

Completes initial assessment of potential residents. Implements service agreements with all residents upon move-in. Completes reviews/revises service agreements as needed and/or required.

Monitors and maintains each resident's medical record, ensures appropriate assessments are completed timely, documentation is appropriate and residents have current care plans in place.

Ensures all physician orders are signed and implemented with-in 24 hours.

Maintains and monitors medication administration program. Supervises medication practices and medical equipment being used in the building. Follows-up with physicians regarding medications and/or medical conditions.

Assists supervisor in providing health and medication training to all new employees as well as continued education for all staff and documents such training to assure compliance with state, federal and local regulations.

Essential Job Responsibilities LPN:

Provides nursing care to residents within the scope of practice of a Licensed Practical Nurse (LPN).

Responsible for coordination and direction of resident care. Coordinates care and services with other departments/providers from the time of admission to discharge.

Assists Registered Nurse with completion of assessments, documentation, and data collection within scope of practice and acting timely on findings.

Administers medications and completes treatments as ordered. Observes and monitors resident's condition and reports changes as appropriate. Ensures that all physicians' orders are signed and implemented within specified time frames.

Maintains safety of all residents and employees. Responds appropriately to safety hazards and reports any issues to the appropriate person. Ensures that all staff follows the infection control policy and procedure.

Maintains a clean, sanitary, safe and efficiently organized work area.

Qualifications

Registered Nurse / Licensed Practical Nurse

Minimum Required Qualifications:

• Graduate from an accredited Nursing program
• Must be licensed as a Registered Nurse or Licensed Practical Nurse within the state in which working and must maintain licensure
• Ability to communicate effectively in both verbal and written formats
• Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

Preferred Qualifications:

• Bachelor's degree
• Senior care experience
• Proven leadership abilities
