CLINICAL CARE SYSTEM LIAISON

Work Location: Work from Home. Must be within a commutable distance to attend in-person meetings as necessary. In-state travel required.

Position Description

This position is responsible to oversee care system product training related to MSHO/MSC+ for internal staff and external customers, delegates and stakeholders. Develop strong partnerships and serve as a key contact for care system and county inquiries about UCare products and care coordination processes including changes in process and regulatory expectations. Responsible for being the key contributor in clinical training of UCare's care system and county partners and ensure that relevant training materials are developed and maintained.

• Develop and maintain a working knowledge of new and existing care system products relative to product functions and introduce new care system products to CLS staff and external customers.

• Serve as the primary contact for care systems and counties. Provide support for clinical and regulatory concerns as they relate to UCare products.

• Coordinate communication regarding care coordination changes. Serve as the lead contact when UCare is outreaching to counties and care systems to discuss care coordination updates.

• Represent CLS in internal and external care system and county-based work groups or initiatives.

• Utilize audit findings to identify knowledge gaps with external customers and assist with closing identified knowledge gaps.

• Maintain a caseload of members as needed by the department.

• Develop and maintain materials for external customers, including training material such as job aids, work instructions, checklists, etc., as well as, product information and updates, care coordination manuals, newsletters, care coordination specific webpages, etc.

• Conduct training as needed for care systems, counties and providers, including model of care training. Provide in-person, live training whenever possible.

• Maintain a strong working knowledge of care coordination functions, requirements and regulations. Subject matter expert in care coordination responsibilities, which includes an understanding of MMIS, activity type assessment results, as well as responding to questions related to care coordination functions independently.

• Coordinate and facilitate care coordination-related projects as directed, such as monthly enrollment files, transitioning large groups of members in response to product or delegate changes, implementing a new product at county sites, responding to information requests by regulators, etc. Be able to prepare and implement project plans, statements of work, and/or process flow related to projects, and be able to prepare and speak to results analysis as needed.

• Collaborate with team and provide back-up as well as maintain a positive working relationship with care system and county staff; maintain those relationships with finesse and graciousness.

• Prepare monthly reports on activities and identify opportunities to increase efficiencies, productivity and quality of work. Be knowledgeable about the demographics that a certain delegate agency works with and be prepared to support them with demographic specific resources, including social determinants of health.

• In-state travel is required.

• Other projects and duties as assigned.

Education

Bachelor's degree in nursing or social work. Current unrestricted Minnesota license.

Required Experience

Two years' experience in training. Strong health plan, managed care, health services, or other related experience. MnCHOICES certified assessor credential in place or with plan to receive assessor credential within 3 months of hire date. Unrestricted driver's license due to training, travel requirements.

Preferred Experience

One-year experience in Medicaid or Medicare risk managed care programs.
