Ecumen Hospice North Branch is hiring a Hospice CNA providing services in Chisago County and East Central Minnesota. The Ecumen Hospice North Branch team focuses on providing life honoring care for patients north of the Twin Cities metro area - including, but not limited to: Isanti, Chisago and Pine counties. Responsibilities

The Hospice CNA is responsible for providing personal care and designated health-related services designed to maintain the client's physical and emotional wellbeing in the home. The Certified Nursing Assistant follows an individualized care plan for the clients they care for.

Essential job responsibilities include:
  • Assists the patient with personal cares which may include all, or some of the following: shampooing hair; bathing using tub or shower; bed bath; sponge bath; rubbing with lotions; fingernail cares; foot care; and other cares as assigned in the care plan.
  • Dresses or assists with dressing, grooming, toileting, oral cares, etc.
  • Assists patient with eating, drinking, walking and transferring.
  • Observes patient and reports changes in physical, cognitive and emotional condition such as change in attitude, loss of appetite, weight, etc.
  • Applies or assists in the use of adaptive equipment which may include but not limited to: hearing aids, glasses, dentures and braces.
  • Qualifications

    Nursing Assistant CNA

    Minimum Required Qualifications:
    • Education: High school diploma or equivalency required
    • Licensure/Certification
    • Current valid driver's license and proof of vehicle insurance
    • Ability to communicate effectively
    • Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident's Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and MSDS.
    Preferred Qualifications:
    • Relevant Experience: One year experience as a CNA/HHA in home health or hospice environment
