RN - TCU Case Manager

DUTIES

The RN Case Manager will provide daily clinical oversight for 20 beds on our Transitional Care Unit. This position will work closely with our interdisciplinary team managing the care and services of each patient from pre-admission through discharge.

QUALIFICATIONS

This position requires RN licensure with the MN Board of Nursing. Must have current CPR certification. Previous experience in a Transitional Care setting is preferred. Must maintain confidentiality to ensure compliance with all HIPAA regulations. Previous supervisory experience preferred. Monday - Friday, Day hours
