ENROLLMENT TEAM SUPERVISOR - SPECIAL NEEDS PLANS

Work Location: Work from Home. Must be within a commutable distance to attend in-person meetings as necessary.

Position Description

As the Enrollment Team Supervisor - SNP, you will be responsible to oversee daily operations of the SNP Enrollment and Verification team. Participate in the hiring and direct supervision of the unable to reach/refusal (UTRR) care coordination, verification, and engagement specialist staff. Mentor, coach, and evaluate individual staff performance. Participate in the planning, implementation, and evaluation of UTRR care coordination, verification, and engagement specialist activities to ensure quality services and compliance with contractual requirements. The team supervisor may be assigned a member caseload at the discretion of the Manager.

• Oversee the performance of care coordination activities including timely performance of engagement and verification outreach, care coordination assessments, development and implementation of care plans, and evaluation of member care outcomes.

• Assign members to staff based on staff caseload size, member needs, geographic location and staff competency. Ensure equal distribution of care coordinator workload.

• Monitor delivery of care coordinator services. Ensure care coordinator records meet contractual requirements. Review and evaluation care coordination staff documentation of assessments, care plans, and care coordination activities, audit member records, monitor staff productivity, identify system barriers and possible solutions.

• Provide training, supervision, and staff development to enhance the knowledge and skills of staff. Train staff on requirements for assigned products and corresponding processes. Establish regularly scheduled one to one staff supervision at a minimum frequency of monthly to review and evaluate appropriateness of services and activities. Establish regular team meetings, assist with problem solving complex member care issues and conduct supervisory oversight of staff/member interactions at least quarterly. This may be via home visit or telephonic member interaction, report quality issues to manager, monitor concerns on the team and ensure confidentiality of all member data, giving input and suggestions for improvement to the manager.

• Participate in meetings as assigned by the Enrollment Team Manager - SNP.

• Provide direct care coordinator services to members based on current department caseload and the organization's business needs. Care Coordinator duties and responsibilities are described in the respective job description.

• Ensure all care coordination regulatory requirements including assessments, care plans, and transition of care activities occur per the product care model and contract.

• Perform operational duties related to specific products as assigned.

• Other projects and duties as assigned.

Education

B.S.in nursing or BA in social work or related field. RN diploma or Associate RN degree plus case management certification may be considered. Current Minnesota license as an RN or licensed social worker required. Required licensure is dependent upon contractual obligations that are determined by assigned product(s).

Required Experience

Three years' experience in care coordination/case management across the continuum of health care (hospital, clinic, nursing home, home care). One year of leadership experience.

Preferred Experience

Experience working with multi-cultural and underserved populations desired.
