Responsibilities

Hospice Nurse Practitioner

The Hospice Nurse Practitioner travels to the hospice patient and perform hospice face to face visits prior to the third and later hospice certification periods by assessing hospice eligibility. The Hospice Nurse Practitioner will provide clinical information to the hospice medical director for interpretation for eligibility and make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.



Essential job responsibilities include:
  • Complies with all state and federal laws and regulations related to providing hospice care.
  • Complies with Ecumen hospice program's policies and procedures, and help Ecumen continually evaluate and improve, when prudent, its policies and procedures. Participates in the hospice program's quality assessment and performance improvement program.
  • Manages symptoms of terminal illnesses by developing an individualized care plan, prescribe medications/treatments, and obtain consultations and appropriate referrals.
  • Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.
  • Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
  • Assumes the medical component of the hospice patient's care program, and assumes responsibility for the palliation and management of the terminal illness and conditions related to the terminal illness.
  • Assesses hospice patients by reviewing the clinical information about the patient and when appropriate, provides documentation that it is anticipated that the patient's life expectancy is 6 months or less if the illness runs its normal course.
  • Prepares, in consultation with the patient's attending physician, if any, and the hospice IDG, and patient or representative, an individualized written plan of care and services for each patient that reflects patient and family goals and interventions based on the needs identified.
  • Qualifications

    Nurse Practitioner

    Minimum Required Qualifications:
    • Graduate from an accredited School of Nursing and accredited Nurse Practitioner Program
    • 1 - 3 years related experience
    • Current nursing licensure in State of Minnesota and CPR certification
    • Accreditation from an approved certifying body for advanced practice nursing
    • Ability to communicate effectively in both written and verbal formats
    • Valid driver's license and proof of vehicle insurance
    • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

    • Prior experience working in Hospice
