ASSOCIATE CARE COORDINATOR
Work Location: Work from Home. Travel required throughout designated counties in Minnesota.
Position Description
This position is responsible to facilitate communication for MSHO/MSC+ members by collaborating with care coordinators, assisting with in-person and telephonic assessments, telephonic communications, data entry, and coordination with contracted vendors. Support Care Coordinators in non-clinical duties to ensure efficient workflow.
• Assist Care Coordinator in the determination of member health care needs by collecting data, participating in in-person and telephonic assessments, and assist with interpreting in order to facilitate the development and implementation of the member's plan of care.
• Provide insight and knowledge into cultural specifics and norms which may affect care delivery to members. This may include work as an advocate to encourage and explain the benefit of preventative and ongoing health care.
• Facilitate the delivery of care and services within the defined benefit set. Identify resources that may be available within the community or at no or low cost.
• Collaborate with Care Coordinators to identify changes in member condition which may warrant reassessment or adjustment to care and services.
• Ensure timely delivery of the following: welcome letters, change of Care Coordinator letters, approvals, mailing of documents, entry of waivered service authorizations, waiver DTRs, and entry of assessments into MMIS to comply with contractual obligations. Responsible for scheduling appointments for assessments and medical appointments, ordering supplies and additional follow up as necessary.
• Must have reliable transportation to travel throughout designated counties in Minnesota.
• Other projects and duties as assigned.
Education
Two-year degree at an accredited college, university, or vocational school with a focus on health, social service, psychology; demonstrated experience may be considered in lieu of degree.
Required Experience
Two years' experience in a community health organization. Familiarity with medical terminology.
Preferred Experience
Experience working with state and federally funded programs. Experience interpreting health information to individuals with varied ethnicity preferred Experience working for an HMO serving members with Medicare and/or Medicaid coverage. Experience in working with culturally diverse populations, underserved populations, or the frail elderly. Bilingual in Hmong, Spanish, Russian, Somali, or Vietnamese.
