We currently have a FT position available for an LSW - new grads or experienced candidates welcome. Join a knowledgeable, supportive, and innovative team that has a great community reputation.

The licensed social worker assists patients to reach their highest level of physical, mental and psychosocial well-being and quality of life.

Job responsibilities: include facilitating admissions; assessments; patient/family advocacy; coordinating care conferences, documentation; service coordination; discharge planning and other duties as assigned.

Qualifications: Bachelor's degree in Social Work and current MN social work licensure; excellent documentation and assessment skills; ability to work independently and as a team member.
