Casual/On-Call Social Services Specialist l 3-5 shifts/month l 8 Hour Day shifts
Ecumen Lakeshore-The Fountains provides short-term rehab and post-hospital care for up to 60 patients in spacious, private rooms along the picturesque shore of Lake Superior. Nurses in the Fountains acquire a comprehensive skill set to include IV insertion and medication administration, lab draws, code response, complex wound care, advanced respiratory care and restorative nursing care in collaboration with the therapy team to support each patient's journey to health.
The Social Services Specialist is responsible for ensuring that residents are linked to the supportive services they need to continue to live independently in their current housing environment. Provides general case management, including intake and referral services to all residents needing such assistance.

Essential Job Responsibilities:
- Establishes linkages with community agencies and service providers to ensure individualized, flexible and creative services for the residents.
- Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual resident.
- Documents activities and provides required reporting, and assists with providing information required for billing.
- Establishes and maintains a directory of providers for use by staff, residents and residents' family members.
- Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate and referring residents to service providers in the community. Helps residents build informal support networks with other residents, family, and friends.
Social Services Specialist Minimum Required Qualifications:
- High school diploma or GED
- Two years of social service experience in a health care setting
- Ability to communicate effectively in both written and verbal formats
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
- Bachelor's Degree in social work
- Licensed Social Worker in good standing in Minnesota
- CPR certification
- Demonstrated continuing education on current trends
- Prior experience working with seniors
