The Home Care Clinical Director ensures that the health and safety needs of all residents are met. This position is responsible for the directing and overseeing the delivery of and monitoring outcomes for skilled and non-skilled services in clients' homes.

Ecumen Home Care-Pathstone is hiring a Clinical Director of Home Care. Ecumen Home Care-Pathstone provides in-home and community-based services in the Minnesota Mankato area.

Clinical Director Essential Job Responsibilities:

1. Plans, coordinates, delivers, and oversees the delivery and outcomes of required healthcare-related services/training programs on a routine basis; monitors both skilled and non-skilled healthcare services.

2. Provides clinical teaching and conducts performance monitoring, identifying and reducing risks

3. Monitors operations to ensure compliance with state licensure and regulations, Medicaid and Medicare standards, as well as state reimbursement regulations.

4. Facilitates communication and serves as a liaison between and among nursing and direct care staff, family and/or guardians, and community medical providers and professional consultants in cooperation with operations management.

5. Directly oversees initial start of healthcare, assessment, and first home visit of all new contracts with clinical component.

6. Directly supervises the implementation, coordination, maintenance, evaluation, and termination of care plan services, analyses outcomes, medical records, and physician orders.

7. Assists in preparation of annual budget for nursing department.

Minimum Required Qualifications:

• Graduate of accredited school of nursing for Registered Nurse.

• 3-5 years related experience

• Licensure/Certification Required: Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.

• Ability to communicate effectively both verbal and written

• Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

Preferred Qualifications:

• BSN degree

• Senior Care experience
