The Intake Specialist position is responsible for the intake and processing of new referrals in accordance with organization standards. This position is responsible for assembling information concerning a patient's clinical background and referral needs, assesses quality of referral documents and provides appropriate clinical information to RN Case Managers for admission.

The Intake Specialist position is responsible for the intake and processing of new referrals in accordance with organization standards. This position is responsible for assembling information concerning a patient's clinical background and referral needs, assesses quality of referral documents and provides appropriate clinical information to RN Case Managers for admission.
Essential job responsibilities include:
- Coordinates the intake of patient information to ensure timely admissions, treatment, interventions and resource utilization in compliance with policies, procedures and regulatory requirements ensuring timely and effective patient care.
- Verifies patient payer sources and schedules all hospice and home care admissions in software system and ensures accuracy of information.
- Responsible for coordination of all patient care; coordinates care and services with other departments/providers from the time of referral to discharge.
- Assumes added responsibility for special projects, administrative functions as directed and follows through with timely completion of the project.
Intake Specialist Minimum Required Qualifications:
- Education: GED or equivalent
- Relevant Experience: Two years of related experience in Medicare Home Care/Hospice
- Current valid driver's license and proof of vehicle insurance
- Ability to communicate effectively
- Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident's Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and MSDS
- Education: Licensed Practical Nurse or Bachelor's Degree in Health Information Management
